Minibus hire from Havana to Vinales

Hi Mario, We have decided to take a horse trek in Vinales and I have tried to complete the booking form on this site to hire a minibus for the round trip from Havana to Vinales but when I click send I get a message “complete all fields”.

I have double checked and completed all the form fields but the message will not go through – can you help?
